Is still wonder if those having problems are just not getting a 'bounce' (giving the reason for the failure to deliver) because 'Refuse Null Sender' is turned on?? Of course, that would not explain why one server can deliver email and be seen in the user account, and another can deliver, but not be seen, in the same user account. That does sound more like a rule/filter/blackhole of some kind.
