Is anything calling a database? This can also happen if a database connection is opened but not closed properly...

Here's an golden oldie, judging by the archives.
I have an exe that calls 3 html files each into it's own little browser created by calling the MS web browser ActiveX.
Each html file loads a shocked asset: .aam, .dcr, .swf. In authoring mode, all is well. When packaged, upon exit I get "the instruction at xxxxxxx referenced memory at yyyyy. The memory could not be "read". Terminate/OK"
I have narrowed this down in that it only happens when the .dcr (shockwave director) file is loaded.
I have packaged this as an exe into the authoring directory and it still crashes every time.
Erick reported that MM tech told him it was a known bug with NT and 5.1 and "they were working on it".
If this is true, could MM confirm this so I can stop tearing my hair out. ;-)
